The World Alzheimer Report 2026 identifies 158 potential drugs across 192 trials. Lecanemab and donanemab offer modest slowing for selected patients, while case reports of amyloid clearance remain weak evidence; prevention and earlier intervention are still unproven. Trontinemab, APOE4 gene-conversion, plasma-proteomic monitoring, and immune, tau, cell, vaccine, vascular, AI, GLP-1, and repurposed approaches remain investigational.
