FDA approved pirtobrutinib for selected frontline CLL/SLL patients without 17p deletion after randomized phase 3 evidence showed an approximately 80% reduction in progression or death versus bendamustine-rituximab. Overall-survival follow-up is immature, and continuous treatment plus infection, bleeding, cytopenia, and cardiac risks will shape implementation.
